The paper considers the case of a 36-year-old man with Marfan syndrome who underwent mitral surgery after personalized external aortic root support operation. Redo surgery was performed without aortic cannulation (with right axillary cannulation and retrograde cardioplegia). Surgical findings revealed unique aortic changes with adventitial growth and vasa vasorum, without visible mesh.
